Understanding Postural Blood Pressure Changes

You stand up from your desk, or rise after a long time on the sofa, and for a few seconds the room swims. You steady yourself on the chair, blink, and carry on. Most of us have done this many times and thought nothing of it.

That moment can have several explanations, and not all of them involve blood pressure. One common one is a brief drop in blood pressure on standing, known as orthostatic or postural hypotension. But doctors interviewed by the Hindustan Times make an important point: dizziness on standing does not automatically mean low blood pressure, and recurring episodes are worth a proper evaluation rather than self-diagnosis. This article explains the best-understood mechanism, the everyday triggers, and, most importantly, when to stop shrugging it off.

What happens when you stand

When you rise, gravity pulls blood down into your legs and belly. Less blood returns to the heart for a moment, so blood pressure dips. Your body has a fast correction system for this: pressure-sensing cells called baroreceptors, located near the heart and in the neck arteries, detect the drop and signal the brain, which tells the heart to beat faster and blood vessels to narrow. All of this happens in seconds, without you noticing.

Dizziness is what it can feel like when that correction runs a beat too slow and the brain briefly gets slightly less blood. The Mayo Clinic notes that such symptoms usually last less than a few minutes, and can include lightheadedness, blurry vision, weakness and, occasionally, fainting.

Everyday factors that can contribute

A brief episode after standing quickly often has an obvious contributor. Common ones include:

Dehydration. Not drinking enough, fever, vomiting, diarrhoea or heavy sweating all reduce blood volume, which makes pressure harder to maintain on standing.

Heat. Hot conditions cause sweating and widen blood vessels.

Long sitting or bed rest. After hours of stillness, or days of illness, the correction system responds more slowly.

Not eating enough, or a large meal. Low blood sugar can be behind occasional lightheadedness, and some people, especially older adults, get a pressure dip after eating.

Alcohol. It raises the likelihood of a drop.

Other pieces of the picture: anaemia (including from low vitamin B12 or folate) can cause similar symptoms, and so can certain medications, including some drugs for blood pressure, heart conditions, depression and Parkinson’s disease. Age matters too, because the baroreceptor response slows over the years.

None of this means you should adjust anything yourself. If you suspect a medication is involved, the right move is a conversation with your doctor, never stopping or changing a dose on your own. And if you have a heart or kidney condition with medical fluid restrictions, follow your clinician’s guidance on fluids rather than general advice to drink more.

When to see a doctor

This is the part not to skip, because dizziness is a symptom, not a diagnosis. The Mayo Clinic’s guidance is clear: see a health care provider if you frequently feel lightheaded when standing, and seek prompt care if you ever lose consciousness, even for a few seconds. Keeping a note of when episodes happen, how long they last and what you were doing makes that appointment far more useful. Mention it especially if episodes ever occur while driving.

The reason frequency matters is that persistent postural blood pressure drops usually point to something else that needs attention, whether that is anaemia, a thyroid or blood sugar problem, a heart condition or a nervous system disorder. Only an examination can sort that out.

Some symptoms call for emergency help, not a routine appointment. The NHS advises calling emergency services if someone who has fainted is not breathing, cannot be woken within a minute, has not fully recovered or has difficulty with speech or movement, has chest pain or a pounding, fluttering or irregular heartbeat, has been seriously hurt, is shaking or jerking, or fainted while exercising or while lying down. Dizziness that arrives with chest pain, breathlessness, palpitations, sudden weakness, speech problems, a severe headache or an unsteady gait is a medical emergency: seek immediate care.

What researchers are still learning

Scientists keep studying this signal because it may carry information beyond the moment. In 2023, the American Heart Association’s news service reported on a large long-term study that found an association between large, early drops in blood pressure on standing and a higher risk of dementia decades later. The researchers were careful to say the link is an association, not proof of cause, and it is not a reason for alarm. It is simply one more argument for something doctors already recommend: if this happens to you often, it is worth mentioning, because these early drops are rarely measured in routine check-ups.

There is gentler news too. A small 2022 study covered by NBC News looked at young women prone to brief dizziness on standing and found that two simple manoeuvres, lifting the knees for half a minute before rising, or crossing and tensing the legs once upright, reduced both the blood pressure drop and the symptoms. The study was tiny and needs confirming, and these manoeuvres were tested on a specific group of young women, not offered as general instructions. The doctors quoted made a wider point: even for a common, usually benign version of this problem, the right first step is a discussion with a physician rather than self-treatment.

Simple habits that ask little of you

For the occasional episode, the habits doctors suggest are unglamorous. Rise in stages: sit at the edge of the bed for a moment before standing. After long sitting, pause on your feet before walking off. Stay hydrated in heat, within whatever limits your own medical conditions allow. Go easy on alcohol, and eat at regular intervals. If you ever feel faint, the NHS advice is to lie down with your legs raised, or sit with your head lowered between your knees, rather than pushing through.

There is a quieter lesson here too: slow, deliberate transitions and close attention to the body’s signals are habits worth building. If you would like to explore yoga and mindful movement, the programs at hellomyyoga are one place to begin.

An occasional head rush is common. A recurring one is a message. Either way, your body is speaking, and it rewards being heard.

Sources and further reading

Research and clinical references

Mayo Clinic, “Orthostatic hypotension (postural hypotension) – Symptoms and causes”: https://www.mayoclinic.org/diseases-conditions/orthostatic-hypotension/symptoms-causes/syc-20352548

NHS (UK), “Fainting” (causes, prevention, and when to seek urgent and emergency help): https://www.nhs.uk/conditions/fainting/

Wieling W., Kaufmann H., Claydon V.E. et al., “Diagnosis and treatment of orthostatic hypotension,” The Lancet Neurology (2022; clinical review): https://www.thelancet.com/journals/lancet/article/PIIS1474-4422(22)00169-7/fulltext

News reporting

Hindustan Times (India), “Everyday Explained: Why getting up too quickly from a sitting or lying position can leave you feeling dizzy” (2026; interviews with Mumbai physicians on causes beyond low blood pressure, fluid-restriction caution, and warning signs): https://www.hindustantimes.com/lifestyle/health/everyday-explained-why-getting-up-too-quickly-from-a-sitting-or-lying-position-can-leave-you-feeling-dizzy-101793343608294.html

NBC News (US), “Dizzy when you stand up?
